What it is used for
Diclofenac relieves pain and reduces swelling and stiffness. It is used for arthritis, including osteoarthritis, rheumatoid arthritis and ankylosing spondylitis, as well as back pain, sprains and strains, gout attacks, period pain, dental pain and pain after surgery. The gel and spray are rubbed on the skin over a painful joint or muscle, and suppositories and injections are used when tablets are not suitable.
How it works
Diclofenac blocks enzymes called cyclo-oxygenases (COX), which make prostaglandins, chemicals that cause pain, swelling and fever. Lowering prostaglandins eases pain and inflammation.

Common side effects
Common side effects of the tablets include indigestion, heartburn, stomach pain, nausea, diarrhoea, headache and dizziness. The gel or spray can cause mild skin redness or itching where it is applied. Taking it by mouth for a long time raises the risk of stomach ulcers and bleeding.
Talk to a doctor or pharmacist first
- People who have had a stomach ulcer, bleeding from the gut, or indigestion with anti-inflammatory painkillers
- People with heart disease, a past stroke, high blood pressure or poor circulation
- People with kidney or liver disease, or who are dehydrated
- People with asthma that gets worse with aspirin or other anti-inflammatory painkillers
- Pregnant women (it should be avoided in later pregnancy) and breastfeeding women
- Adults over 65, and anyone taking blood thinners, steroids, SSRI antidepressants or blood pressure medicines
Warnings
- Stop and get urgent help if you vomit blood, pass black or bloody stools, or have chest pain, weakness on one side of the body or trouble speaking.
- Do not take it with other anti-inflammatory painkillers such as ibuprofen, naproxen or aspirin for pain.
- Avoid it if you might have dengue, as it can increase the risk of bleeding. Paracetamol is usually used for fever in dengue.
- Use the lowest amount that works for the shortest time, as your doctor advises.
- Keep the gel away from the eyes, mouth and broken skin, and wash your hands after applying it unless you are treating your hands.

For pain in joints close to the skin, such as the knees and hands, the gel can relieve pain while much less of the medicine reaches the bloodstream. This means fewer stomach side effects, although the gel is not suitable for every kind of pain.
