What this test is for
Ovulation & pregnancy hormone
Progesterone is a hormone made mainly by the ovaries after ovulation. It prepares the lining of the womb for a possible pregnancy and helps support early pregnancy. Because progesterone rises clearly only after an egg has been released, a blood level taken at the right time in the cycle helps your doctor check whether ovulation has happened.
Why a doctor may order it
- To check whether you are ovulating when trying to get pregnant
- To look into irregular periods or difficulty conceiving
- To monitor some fertility treatments
- In early pregnancy, when there are concerns such as bleeding or pain, alongside other tests
How to prepare
Fasting: Not needed.
There is no need to fast or prepare in any special way. Timing is important: to check ovulation, the sample is usually taken about a week before your next period is expected, so book the collection on the day your doctor advises. Mention any hormone medicines, including progesterone supplements, to your doctor.

What the results can mean
Progesterone is reported with ranges for different phases of the cycle and for pregnancy. A clearly raised level in the second half of the cycle suggests that ovulation has taken place, while a low level may mean ovulation did not happen that month or that the sample was taken at the wrong time. Cycles vary, so the test is sometimes repeated. Your doctor will interpret the result with your cycle dates and other tests.

Questions people ask
When should I have a progesterone test?
To confirm ovulation, it is usually taken about seven days before your next period is due. In a regular 28-day cycle this is around day 21, which is why it is often called the day 21 progesterone test.
Does one low progesterone result mean I cannot get pregnant?
No. Ovulation does not happen in every cycle for everyone, and a sample taken too early or too late can show a low level. Your doctor may repeat the test in another cycle.
Why is progesterone checked in early pregnancy?
In some situations, such as bleeding or pain in early pregnancy, progesterone can help doctors assess the pregnancy alongside a scan and hCG tests. It is not used on its own to make decisions.
