What this package is for
Men's cancer markers
This panel combines four tumour markers with a blood count and ESR. PSA relates to the prostate, AFP mainly to the liver, and CEA and CA 19-9 to the digestive system, including the bowel and pancreas, while the blood count and ESR add general information about anaemia and inflammation. Doctors use these markers mainly to support the assessment of a suspected cancer or to monitor one that has been treated, not as a screening test for healthy men.
Who this package is for
- Your doctor is investigating symptoms such as weight loss, anaemia or a change in bowel habit
- You are being followed up after cancer treatment and your doctor tracks marker levels
- You have long-term liver disease, such as hepatitis B, and your doctor monitors AFP alongside scans
- You have urinary symptoms and your doctor wants a PSA as part of the assessment
How to prepare
Fasting: Needed; you are told how long to fast when you book, and plain water is fine.
Fasting is needed before this sample; you are told how long to fast when you book, and plain water is fine. Avoid vigorous cycling and ejaculation for about two days beforehand, as both can raise PSA for a short time. Keep taking your usual medicines unless your doctor says otherwise, and tell your doctor if you take medicines for an enlarged prostate, as some of them lower PSA.
At home
- Sample
- Blood sample
- How it works
- Book a morning slot so the collector can take the sample before you eat. A small amount of blood is drawn from a vein in your arm, which takes a few minutes, and it is used for both the markers and the blood count.
- Report
- Within 24 hours of collection.
What the results can mean
A raised marker is not a diagnosis of cancer. PSA can rise with an enlarged or inflamed prostate, AFP and CA 19-9 with liver or bile duct disease, and CEA in smokers, while a normal result does not rule cancer out. The blood count and ESR may show anaemia or inflammation that needs its own explanation. Your doctor will interpret the results with your symptoms, examination and any scans, and may repeat a result before acting on it.

- Prostate-Specific Antigen (PSA), TotalPSA is a protein made by the prostate, a small gland in men that sits just below the bladder.
- Free PSA (Free Prostate-Specific Antigen)PSA is a protein made by the prostate gland.
- Carcinoembryonic Antigen (CEA)CEA is a protein normally present only in very small amounts in adults' blood, but some cancers, particularly bowel (colorectal) cancer, can release more of it, so it is known as a tumour marker.
- CA 19-9 (Cancer Antigen 19-9)CA 19-9 is a tumour marker, a substance that some cancers release into the blood.
- Alpha-Fetoprotein (AFP)Alpha-fetoprotein is a protein made mainly by the liver of an unborn baby, so levels are naturally high in pregnancy and very low in other adults.

Questions people ask
Can this panel screen for cancer in healthy men?
Not reliably. These markers miss many early cancers and can be raised by harmless conditions, so they are not advised for routine screening without symptoms. PSA testing in particular should be discussed with a doctor first.
What does a high CEA mean?
CEA can be raised in bowel and some other cancers, but also in smokers and in people with inflammation of the bowel, liver or lungs. It is most useful for monitoring a cancer that has already been diagnosed.
Why is a blood count included with cancer markers?
A blood count can show anaemia, which may be a sign of slow bleeding from the gut, and the ESR gives a general measure of inflammation. Both help your doctor decide what to look at next.
What is AFP used for in men?
AFP is used mainly to help assess liver cancer, especially in people with long-term liver disease, and some types of testicular cancer.
