What this test is for

Prostate health & cancer screen

PSA is a protein made by the prostate, a small gland in men that sits just below the bladder. A small amount of PSA normally passes into the blood. The level can rise with prostate cancer, but also with common non-cancer conditions such as an enlarged prostate or a prostate infection, so the test is used as a first check that may lead to further tests.

Why a doctor may order it

  • If you have urinary symptoms such as a weak stream, difficulty starting or passing urine often at night
  • To check the prostate in older men, after discussing the benefits and limits with a doctor
  • If there is a family history of prostate cancer
  • To monitor men who are being treated or watched for prostate cancer

How to prepare

Fasting: Not needed.

Avoid ejaculation and vigorous exercise, especially cycling, for two days before the test, as these can raise PSA for a short time. Tell your doctor if you have a urine infection, have recently had a prostate examination or procedure, or take medicine for an enlarged prostate, as some of these medicines lower PSA. You can eat and drink normally.

How it works
A trained collector takes a small blood sample from a vein in your arm in the privacy of your home. The draw takes a few minutes.
Report

What the results can mean

The report shows your total PSA next to the laboratory's reference range, which may vary with age. A raised PSA does not mean you have cancer: an enlarged prostate, infection, recent ejaculation or exercise can all raise it, and some prostate cancers do not raise PSA. Your doctor will interpret the result and may repeat it, check free PSA or suggest further tests.

What's included

One test: Prostate-Specific Antigen (PSA), Total, from one blood sample.

Questions people ask

Does a high PSA mean I have prostate cancer?

No. Most men with a raised PSA do not have prostate cancer. An enlarged prostate and infection are common causes. Your doctor will decide whether further tests are needed.

What is the difference between total PSA and free PSA?

Total PSA measures all the PSA in the blood. Free PSA measures the part not bound to proteins. The ratio of free to total PSA can help your doctor judge whether a raised PSA is more likely due to cancer or an enlarged prostate.

Who should have a PSA test?

Men with urinary symptoms, a family history of prostate cancer, or concerns about prostate health can discuss the test with a doctor. Because it has benefits and limits, it makes sense to decide together with your doctor.
