Ankylosing spondylitis risk
HLA-B27 is a protein found on the surface of white blood cells in some people, and whether you have it is decided by your genes. It is strongly linked with ankylosing spondylitis, a type of inflammatory arthritis of the spine, and with related conditions such as reactive arthritis and some kinds of eye inflammation. The test helps your doctor judge how likely these conditions are, but it cannot diagnose them on its own.

- Long-term back pain and stiffness that is worse in the morning and eases with movement
- Back or buttock pain that started in a young adult
- Repeated episodes of a painful, red eye (uveitis)
- Joint pain and swelling after a gut or urinary infection
- A family history of ankylosing spondylitis
How to prepare
Fasting: Not needed.
Nothing special is needed before this test; you can eat, drink and take your usual medicines as normal. Because HLA-B27 is inherited and does not change, this test normally only needs to be done once.

What the results can mean
The result is reported as positive or negative. A positive result makes ankylosing spondylitis or a related condition more likely when you have typical symptoms, but many people with HLA-B27 never develop any of these conditions. A negative result makes them less likely but does not rule them out. Your doctor will interpret the result with your symptoms, examination and scans.
