What this test is for
Rheumatoid arthritis check
This blood test looks for antibodies against cyclic citrullinated peptides, which are found mainly in people with rheumatoid arthritis. Because these antibodies are rarely present in other conditions, a positive result strongly supports a diagnosis of rheumatoid arthritis. The test is often ordered together with rheumatoid factor, and the antibodies can sometimes be found before the disease has fully developed.
Why a doctor may order it
- Painful, swollen joints, especially in the hands, wrists or feet
- Joint stiffness in the morning that lasts a long time
- To help diagnose early arthritis when other tests are unclear
- When symptoms suggest rheumatoid arthritis, whatever the rheumatoid factor result
How to prepare
Fasting: Not needed.
You can eat and drink as usual, as this test does not need fasting. Keep taking your regular medicines, including painkillers or arthritis treatment, unless your doctor tells you otherwise.

What the results can mean
A positive anti-CCP result makes rheumatoid arthritis more likely, particularly with joint swelling and a positive rheumatoid factor. Higher levels have been linked with a greater chance of joint damage, which is one reason early assessment matters. A negative result does not rule out rheumatoid arthritis. Your doctor will interpret the result with your symptoms, examination and other tests such as ESR and CRP.

Questions people ask
What is the difference between anti-CCP and rheumatoid factor?
Both are antibodies linked to rheumatoid arthritis. Rheumatoid factor can also be positive in other conditions and in some healthy people, while anti-CCP is more specific, so doctors often use the two together.
Can I have rheumatoid arthritis with a negative anti-CCP?
Yes. Some people with rheumatoid arthritis have neither anti-CCP nor rheumatoid factor. The diagnosis is based on symptoms and examination as well as blood tests.
Does a positive anti-CCP mean I will get rheumatoid arthritis?
Not always. Anti-CCP can appear before symptoms start, but not everyone with a positive result goes on to develop the disease. Your doctor will advise on follow-up.
