Inherited enzyme deficiency (G6PD)

G6PD (glucose-6-phosphate dehydrogenase) is an enzyme that protects red blood cells from damage. Some people inherit a low level of this enzyme, known as G6PD deficiency, which is more common in men. In these people, certain medicines, fava beans or infections can cause red cells to break down suddenly, leading to anaemia and jaundice. This test measures how much G6PD activity your red cells have.

Why a doctor may order it

  • Jaundice or dark urine after certain medicines, fava beans or an infection
  • Before medicines that can cause red cell breakdown in G6PD deficiency, such as primaquine for malaria
  • Newborn jaundice that is severe or lasts longer than usual
  • A family history of G6PD deficiency
  • Unexplained anaemia due to red cell breakdown (haemolysis)

How to prepare

Fasting: Not needed.

You do not need to fast. If you have recently had an episode of red cell breakdown or a blood transfusion, tell your doctor, because the result can look falsely normal for a while afterwards and the test may need repeating a few months later.

What the results can mean

The report shows your G6PD activity compared with the laboratory's reference range. A low level suggests G6PD deficiency, which is lifelong but often causes no problems as long as known triggers are avoided. Women can have intermediate levels. Your doctor will interpret the result and, if you have the deficiency, can explain which medicines and foods to avoid.

What should people with G6PD deficiency avoid?

Common triggers include fava beans, mothballs containing naphthalene, and some medicines, including some antimalarials and certain antibiotics. Always tell your doctor and pharmacist that you have G6PD deficiency.

Why is G6PD deficiency more common in men?

The gene is carried on the X chromosome, so men with the gene are affected, while women are more often carriers, although some women are also affected.

Can the test be misleading after a haemolytic episode?

Yes. After red cells break down, the younger red cells that replace them have more of the enzyme, so the result can look normal. Your doctor may recommend repeating the test a few months later.
