What this test is for
Folate level, anaemia & pregnancy
This test measures folate, a B vitamin (vitamin B9) found in green leafy vegetables, pulses and fortified foods; folic acid is the form used in supplements. The body needs folate to make healthy red blood cells and new cells, and it is especially important in early pregnancy for the baby's developing spine and brain. A low level can cause anaemia with large red cells.
Why a doctor may order it
- Anaemia with large red blood cells, often checked alongside B12
- Tiredness, weakness or a sore tongue
- Poor diet, heavy alcohol use or bowel conditions that reduce absorption, such as coeliac disease
- Before or during pregnancy when folate status is a concern
- Long-term use of medicines that affect folate, such as some epilepsy medicines or methotrexate
How to prepare
Fasting: Not needed.
There is no need to fast. Eat as you normally do, but let your doctor know about folic acid, multivitamins or other supplements, because recent intake can raise the level. Keep taking prescribed medicines unless your doctor advises otherwise.

- How it works
- A small blood sample is taken from a vein in your arm by a trained collector at your home. It takes a few minutes, and the sealed, labelled tube is sent on to the laboratory.
- Report
- Within 2 days of collection.
What the results can mean
Your folate result is shown with the laboratory's reference range. A low level suggests the body is not getting or absorbing enough folate, or is using more than usual, for example in pregnancy. Blood folate reflects recent diet, so one low result may not show long-term stores. Because B12 deficiency can look similar, your doctor will usually check B12 too before deciding what the result means.

Questions people ask
Why is folate important in pregnancy?
Folate helps the baby's spine and brain develop in the first weeks of pregnancy. Low folate increases the risk of conditions such as spina bifida, so folic acid is commonly advised before and in early pregnancy. Ask your doctor what is right for you.
What is the difference between folate and folic acid?
Folate is the natural form found in food. Folic acid is the man-made form used in supplements and fortified foods. Both are forms of vitamin B9.
Should B12 be checked with folate?
Usually, yes. Both deficiencies cause a similar anaemia, and treating low folate alone when B12 is also low can let nerve damage from B12 deficiency go unnoticed.
