How to prepare
Fasting: Not needed.
You can eat and drink normally before this test. If you take thyroid medicine, ask your doctor whether to have your blood taken before that day's tablet, and do not change how you take it without their advice. Biotin supplements can give misleading thyroid results, so tell your doctor if you use them.

What the results can mean
If TSH is high and free T4 is low, the thyroid is likely underactive; if TSH is low and free T3 or free T4 is high, it is likely overactive. A mildly raised TSH with normal hormone levels, called subclinical hypothyroidism, is common and often just needs a repeat test.
